Heritage Baptist Church Vacation Bible School (VBS)

Permission & Liability Waiver

1. Medical Authorization

I authorize the staff, volunteers, and adult workers of Heritage Baptist Church Vacation Bible School to administer basic first aid and, if necessary, obtain emergency medical treatment for my child. This includes transportation to a medical facility and treatment deemed necessary by licensed medical personnel.

I understand that every reasonable effort will be made to contact me before treatment is provided. However, in the event I cannot be reached promptly, I give permission for emergency medical care to be administered.

2. Release of Liability

I understand that participation in Vacation Bible School activities may involve certain inherent risks of injury. In consideration of my child being allowed to participate, I release and hold harmless Heritage Baptist Church, its pastors, staff, volunteers, and representatives from any claims, liabilities, damages, or causes of action arising from my child’s participation in VBS, except where prohibited by law.

3. Assumption of Risk

I voluntarily permit my child to participate in all scheduled Vacation Bible School activities, including indoor and outdoor games, crafts, recreation, Bible lessons, and other supervised activities. I acknowledge that accidents may occur despite reasonable precautions and accept the risks associated with participation.

4. Photo & Video Release

I grant permission for Heritage Baptist Church to photograph or record my child during Vacation Bible School. I understand these photos or videos may be used in church publications, promotional materials, presentations, social media, or on the church website without compensation.

5. Behavior Agreement

I understand that my child is expected to follow the rules established by Heritage Baptist Church Vacation Bible School and to demonstrate respectful, Christ-like behavior toward leaders and other children. I understand that repeated misconduct may result in my child being dismissed from VBS if deemed necessary by church leadership.
